Care Delivery:
Advanced medical care providers direct access to the assigned medical providers, 7 days per week, 364 days per year. Our medical team is equipped to handle medical needs in the home or urgent care settings that may have otherwise required emergency room visits or inpatient hospitalizations.
Scheduling:
Contact our office and care will be coordinated 7 days per week/364 days per year. Our highly trained medical team will respond in a timely manner to coordinate urgent care needs and whenever necessary a Hous Call Visit will be made within 24 to 72 hours of request for Service.

Tier I: Acute & Urgent Medical Care at the "Residence"
Provision of medical assessment and treatment to our patients at their residence
Provision of follow-up care to our patients’ at their residence
Coordination of transfer of our patients to a higher level of acute care(should it become necessary)
Tier II: Acute & Urgent Medical Care at the TrinityXpressMed Medical Centers
Immediate medical access to our Urgent Care centers for all patients
No appointments are needed
TrinityXpressMed Medical Centers are conveniently located in Fort Worth and the Mid Cities
TrinityXpressMed Medical Centers are open 7 days/week, 364 days/year
TrinityXpressMed Medical Centers fully staffed by Board Certified physicians and other highly credentialed medical providers
Availability of on-site x-ray, laboratory, cardiac and pulmonary diagnostic services
TrinityXpressMed Medical Centers avoid unncessary ER visits
TrinityXpressMed Medical Centers provide proper medical care at a lower expense
Tier III: Acute Emergent Medical Care
Coordination of proper and timely transfer of our patients to an appropriate ER
We do not, however, offer direct medical oversight in the ER
Coordination of hospital admission for our patients (should it become necessary)

Tier IV: Primary care & Chronic Medical Care
Provision of routine annual physicals and health maintenance services to our patients
Provision of post acute care follow-up visits
